Have you ever served in the military? Providing a practical tool to community healthcare providers who may encounter Veterans receiving community care. A writing pen with an attached pull-out banner with an imprinted, VHA constructed, QR code that can be scanned utilizing any smart phone to assist community healthcare worker to identify Veteran care needs. The questionnaire provides guidance on how to ask general questions to elicit responses as well as information regarding Compensation & Benefits, Living Situations, Unwanted Sexual Experiences in the Military, Exposure Concerns, Behaviors, Blood-Borne Viruses, Health Risks Associated with Specific Eras, and Common Service-Related Exposure Concerns. The brochure also lists the VA Crisis Line phone number as well as websites for www.myhealth.va.gov and www.publichealth.va.gov/exposures. By asking 1 Question, community providers can assist the VA in helping serve those who have served!

Automatic Inactivation of Unused Opioids

Current opioid disposal options are plagued by low participation rates resulting in the accumulation of millions of unused opioids in the community. These unused opioids are a primary exposure event for people who develop new opioid addictions and experience opioid-related adverse events such as overdose. Reducing this source of opioids can reduce the harm of unprescribed opioid exposure.
Our innovation involves the use of a novel pill bottle cap technology. Our system automatically inactivates unused opioids, after the prescription window ends, through the timed release of a modifying/inactivating agent contained within the pill bottle.
All existing options for opioid disposal require patient participation and are plagued by low participation rates. Our project design addresses this “Participation Gap” by incorporating automaticity.
By functioning automatically, our device will increase disposal rates and reduce the number of unused opioids available for misuse, abuse, and diversion. Our cap is compatible with existing industry standard medication bottles.

Blood Pressure Support Frame

Hypertension is known as the silent killer. The CDC estimates 36,524 per year from hypertension which is preventable and treatable.
Getting a blood pressure reading can be difficult for people who are unable to apply a blood pressure cuff to themselves. Commercial grade units, often found near pharmacies, are too big and expensive for private use in the home. This is the reason for the development of the Blood Pressure Support Frame. A unit that can hold the cuff open so the patient can slide their arm into the cuff and it will allow the cuff to tighten and get a reading without help.
With the ability to monitor the blood pressure at home the patient it will allow for reduced stress and a more accurate reading. Testing before and after medication assesses the effectiveness of the medication without frequent hospitalizations or trips to the providers office to be checked.

Building Confidence Builds Psychological Safety

Problem: As a soft-skills educator, I fear that in-person role plays we do not yield a high return on learner ability, for our investment in coursework.
How might we use virtual reality to provide employees with safe spaces to practice high-stakes, stressful conversations?
In FY21, we conducted human-centered design evaluations, in which learners conversed with virtual humans, who were controlled by artificial intelligence. Learners’ reactions were positive. However, the FY21 VR conversations were off-the-shelf.
Innovation: Author our own VR content and test learner reactions when they are immersed in scenarios which are not canned but instead are highly relevant to their local, daily workplace.
We found that obtaining an authoring tool is fraught with complications.

Clinical Stimulation-Driven Rowing Program for Veterans with Paralysis

Veterans with paralysis are often extremely sedentary leading to profound deconditioning, premature aging and many secondary health complications. Individuals may also have weakness of muscles responsible for respiration. Eighty percent of all individuals with paralysis have respiratory complications. This is the number one cause of death in this population.

Air Pillow on a Roll (APOAR)

Innovation can be a complicated process or simple as revising something we use every day. Healthcare providers faced challenges over the past few years, living through and caring for people during a PANDEMIC! Circumstances that have everyone looking through different lenses to provide best care practices.
Collaborating with i-Net VAMC in Cincinnati, Suzanne Zwerin, PACU RN, has pivoted, iterated and prototyped APOAR (air pillow on- a-roll). A positioning device to improve patient satisfaction, reduce potential infections, create smaller waste, optimize storage space and provide caregivers quick solutions for obtaining a positioning pillow.
APOAR can be issued when patients seek hospital care, both inpatient and outpatient settings. The air-filled pillow is issued from a roll-out station, available in each room or department. The wall mounted dispensary houses the pillows on a roll and user-friendly source of inflation. Best of all veterans can take their pillow home for further healing and recovery.

EAT at Home: Evaluation and Treatment of Homebound Veterans with Cardiovascular Conditions using Innovative Technology

Evaluation of QT prolongation, bradycardia, tachycardia, and atrial fibrillation is more difficult for the homebound veteran. As a result, health care discrepancies occur between ambulatory veterans who can come into the VA for diagnostics and those with a homebound status. Cardiovascular is data needed to assess if a veteran needs to report to the emergency department (ED), improve safe medication prescribing, evaluate polypharmacy and the impact of QT prolongation and subsequent sudden cardiac death, and assessment of palpitations and atrial fibrillation. EAT (evaluation and treatment) at home is a pilot project aimed to address this health care gap. Home-based primary care (HBPC) staff will use a survey to assess if veterans qualify for an apple watch, athletic wearable watch, Kardia mobile device (portable ECG), vital sign equipment with paper sheets for recording, or telehealth. This pilot project will occur over a six month period to explore how innovation can be used in the home to improve health care outcomes and quality of life.

Employee Well Being Center and Carts

Employee Well Being Center and Carts: A High Reliability Organization Opportunity
The Employee Well Being Center and Carts offer a Whole Health approach to self-care using methods from the Circle of Health. Almost every experience available in the Well Being Centers can be duplicated on the Well Being Carts for the staff, as well as providing them a unique immersive experience with the VR headsets. The VR headsets enable the staff member to virtually “leave” their work area and escape to a tropical beach, a snow-covered vista in the mountains, swim with dolphins or enjoy a serene mindfulness exercise, all while seated at their own workspace! The Employee Well Being Centers and the Well Being Center Carts impact employees on a personal and systemic level by:

Help Is Only A Phone Call Away

We’ve developed a quick phone guide for Veterans. It is a 3x5 dry erase magnet that provides all the necessary information for a Veteran during a crisis situation. The magnet can be posted in a familiar area such as their refrigerator or bathroom medicine cabinet. This is a quick and easy way to access their provider should they need to refill medication or make an appointment. We’ve developed this magnet to be the size of a 3x5 index card so that the font is large enough for Veterans to see. We also designed it to be erasable so that the numbers can be changed should the Veteran’s provider moves to another department, or if the Veteran changes providers.

iESP-Individualized Emotional Stability Plan

People in an emotional crisis or life-altering event often react from fear, become immobilized, and don’t know what to do. Ineffective coping skills and loss of emotional stability can make the situation more difficult and last longer with greater negative consequences. As less than 10% of adults see a therapist, my question is: What tool can be put into people’s hands to help them remember their personal strengths and supports to manage better? iESP incorporates internal supports, external actions, friends, family, and professionals to keep someone from becoming frozen in the moment, unable to cope or strategize, and offers individualized steps to move through the challenges they are experiencing. iESP is a personalized protocol that can be quickly filled out, and reminds one of their personal strengths and preferences during a difficult time. iESP does not replace therapy; it is a another personalized tool in an individual’s emotional stability arsenal.

Improving Healthcare Access for Veterans with a Serious Mental Illness (SMI)

As the Local Recovery Coordinator for the VA Southern Nevada Healthcare System, one of my responsibilities involves re-engaging veterans diagnosed with a Serious Mental Illness (SMI) back into care. In the process of talking with veterans to re-engage them into care, I realized there were some common threads as to why veterans were dropping out of care. This led me to designing a proactive approach to keeping Veterans engaged in care. Initially I focused on a Box-5 solution for this problem and created a Clinical Reminder but was soon introduced to the concept of human centered design and encouraged to apply for a Spark Challenge through the Spark, Seed, Spread Innovation Investment Program. My focus throughout this challenge has been on developing proactive means for keeping veterans with an SMI engaged in care because routine treatment improves Quality of Life, helps with recovery, and prevents suicides.

Infection Precaution Door Barrier (IPDB)

Despite the provision of Personal Protective Equipment (PPE) to staff, deviations from recommended practice, procedure, or standards are common. Well meaning staff enter a patient’s room to speak with them and/or family members, check devices, or drop something off with no intention of touching the patient or items in the room. But these encounters often result in contact with the patient or equipment which can spread to the next patient, other staff, and even staff family members when they go home. I want to use a type of door barrier that will serve as a physical reminder to staff that PPE is required every time, they enter an isolation precautions room, as well as a reminder to take off all PPE before exiting that insolation precautions room.

In-House Eyeglass Fabrication for Blind Rehabilitation

The expected wait time for prescription eyeglasses from a VA optical shop is 4-6 weeks. The average length of stay for a legally blind Veteran at a VA blind rehabilitation center is 28 days. These Veterans don’t receive their glasses during their training program to troubleshoot and evaluate effectiveness with a specialist. Instead of outsourcing the prescription eyeglass orders for visually impaired Veterans, this program makes these glasses in-house at the VA blind rehabilitation center. Doing this significantly reduces the turnaround time between order and issuance of the eyeglasses and allows for better assessment of needs and a reduction in unused prosthetics. This program can also provide the same service to outpatient low vision clinics within the blind rehab center’s VISN or catchment area. Reducing the amount of travel required for these Veterans to obtain their glasses and allow them to use them during their outpatient programs.

Linking Veterans Who Inject Drugs into Harm Reduction Services

Syringe services programs are community-based prevention programs which provide evidence-based, life-saving services for people who use illicit drugs, including access to syringes, naloxone, fentanyl test strips, infection screening and linkage to treatment. Historically, syringe services programs did not exist within the Veterans Health Administration due to many factors. At the VHA facility in Orlando, Florida, the infectious disease team observed increasing HCV reinfection rates. We implemented the RIS4E program (Resists Infection by Sterile Syringe, Safe Sex & Education) in order to link Veterans who use illicit into VA services.

Low Vision Tele-eye Rehabilitation

Veterans with low vision who live in rural communities have limited access to VA Low Vision Optometry Rehabilitation Services, unless they are able to travel several miles to a Low Vision Clinic. When you add the loss of the ability to drive, travel becomes an insurmountable obstacle to receiving Low Vision Services. Low Vision TeleEye Rehabilitation clinical video telehealth (CVT) currently allows Low Vision Rehabilitation with a Low Vision Optometrist and Therapist in a timely manner in their home, at a CBOC or rural VA.
Veterans with low vision can be scheduled for in-person Low Vision Rehabilitation Services at the Buffalo VA’s Vision Impairment Services for Outpatient Rehabilitation clinic. Partially sighted or legally blind Veterans who cannot travel to the Buffalo VA have the option to schedule Low Vision TeleEye Rehabilitation CVT in their home and at 3 rural VA’s and 10 Community Based Outpatient Centers in Western New York.

TST-VR enhances/expands the reach of TST by creating a VR platform for TST homework. TST-VR has invaluable therapeutic potential as outside of therapy session practice is a key factor in treatment success. Smart TMS System

Transcranial magnetic stimulation (TMS) is a safe and effective treatment for depression in those who have not responded to medications. It utilizes an electromagnetic coil that is placed over the scalp to stimulate targeted areas of the brain. Proper coil positioning is critical, as small errors in coil positioning can greatly reduce the strength of the stimulation. In this project, we have designed a "smart" TMS coil which uses sensors and displays to provide feedback to the clinician to optimally position the coil for the duration of each treatment, which can last up to 40 minutes, and repeated daily ~30 sessions. We have demonstrated that using the smart coil, accuracy is superior to conventional methods of coil placement, and comparable to large research grade setups costing >$75,000. Building on this technology, we have developed a closed-loop prototype. The ultimate goal is to utilize this technology in at-home setting to greatly accessibility of the treatment.

Volunteers often miss emails due to sporadic use or emails go to spam folders. Most people don’t answer calls from unrecognized numbers. Each effort at communication, especially when a response is needed, requires several emails and calls to communicate with volunteers about just one issue. Text to Serve is an application software that will allow us to communicate to groups of volunteers via push notifications or text. Anesthesiology and Perioperative Outcome Data Analysis

Analysis of anesthesiology data combined with other perioperative datasets present an opportunity for major improvements in the field. Anesthesiology extends beyond the operating room through preoperative and postoperative activities and variations in practice can have a large impact on patient outcomes. Anesthesiologists apply a wide variety of measure-based improvement work to their practice. Having readily available, high-quality data is critical to foster and support these activities. VA’s current ecosystem of isolated, disparate Health Information systems and lack of availability of standardized perioperative quality and outcomes measures limits our ability to perform data-driven improvement work at large scale. This project aims to integrate anesthesiology case data with other VA datasets and provide a standardized set of perioperative quality measures across the enterprise to enable better field-based projects, provide an evaluation of interfacility variability to highlight best practices across VA, as well as grant direct comparisons between VA and community healthcare institutions.

The Department of Veterans Affairs Office of Healthcare Innovation and Learning (14HIL) Discovery, Education and Affiliate Networks (14), SimLEARN (14HIL2), created a virtual representation (digital twin) simulation system for real-time collaboration, creation, and dissemination of simulation content, scenarios, and best practices across the Veteran Health Administration’s (VHA) 170 medical centers.
The simulation system is used to support planning the design and workflows for existing and new healthcare facility spaces. The software application includes the following capabilities:

Musculoskeletal (MSK) Telehealth Program

In the past decade, MSK conditions were identified as the most frequent diagnoses in primary care clinic encounters in the US. Due to the COVID-19 crisis and an increase in provider telehealth appointments the SME’s practicing MSK, identified a gap in Veteran care. The purpose of the SimLEARN MSK Telehealth virtual simulation training course is to provide clinicians with the framework necessary to perform telehealth shoulder and knee examinations in a changing healthcare environment. Learners review shoulder and knee complaints, practice using the verbal instructions delivered to Veterans before and during the virtual MSK physical examination and are able provide a diagnosis based upon virtual observations. Clinicians master their techniques in a safe environment and are provided specific guidelines both written and visual to enhance their knowledge when evaluating the shoulder and knee in the telehealth environment. The goal of the program is to improve knowledge, skills and confidence to manage common shoulder and knee problems that affect our Veteran populations. In addition, this training emphasizes appropriate resource utilization and access to specialty care. “Connecting Veterans to the soonest and best care.”
